The West London NHS Trust rough sleepers service plays a key part in responding to the needs of rough sleepers, who have mental health and physical health difficulties. The service works across Hammersmith and Fulham, Ealing and Hounslow - with a range of partners, including St Mungo's outreach service and local authority homelessness departments.
The team includes:
- A consultant psychiatrist
- A speciality psychiatrist
- Community psychiatric nurses
- Occupational therapist
- Social workers (including approved mental health professionals - AMHP)
We work flexible hours which include late night and early morning shifts that enable us to access those who need the service at times that are most helpful to them.
We assess mental and physical health needs, and help rough sleepers to connect with relevant health and appropriate accommodation services.
We work in shifts and in partnership with our outreach partners, St Mungo's and Thames Reach.
The team also provide some in-reach support to those rough sleepers who are in temporary accommodation such as hostels or hotels.
