• We help people manage long term conditions and provide community nursing, often at home. These services include management of diabetes, bladder and bowel problems, wound care and home oxygen.

  • We help people avoid admission to hospital with our Rapid Response and Home First teams, and speed up their recovery in specialist community-based rehabilitation wards

  • We help people with ongoing conditions to retain and improve their independence. These services include: physiotherapy, podiatry, speech and language therapy and occupational therapy.

  • Adults with a diagnosis of learning disability can get support from the community team for people with learning disabilities (CTPLD). This team of health and social work professionals work in partnership with people with learning disabilities, their families, carers and other agencies.

  • Our psychological medicine services are part of Ealing Community Partners (ECP). They work alongside and are integrated with our other community teams. We support people with psychological needs caused by, or contributing to, long-term physical health conditions.

  • We provide a range of support services for people diagnosed with dementia, which also provide respite for carers.

  • We offer a range of support and advice services to help you stay well. These include tuberculosis (TB) awareness and support for patients to stop smoking, and with other ways of improving health.

  • We provide proactive outreach and remote clinical support to certain residential care homes in Ealing, speeding up communication with patients’ registered GPs and other services.

  • The community independence service (CIS), in partnership with Central and North West London NHS Foundation Trust, offers care at home to help people avoid unplanned hospital admissions.

  • 111*6 is a clinical telemedicine advice line for care homes across North West London. This service offers a dedicated *6 telephone line that allows care homes to get a quicker response and clinical advice when calling NHS 111. There’s an option for some care homes to convert to a video call for additional support.

  • We provide pharmacy services at Clayponds Hospital, Broadmoor Hospital and the St Bernard’s Hospital site.The pharmacy service supports staff, patients and carers to use medicines safely and effectively.

  • The safe spaces are local hubs for anyone, including carers who feels they’re nearing crisis point. Staff are available to listen and work with you to help. The service is provided in partnership with Hammersmith and Fulham, Ealing and Hounslow Mind.
