Support, enable and empower people with learning disabilities to become active and healthy citizens, by living socially valued and inclusive lives and to help people be accepted and respected for who they are.

The Hounslow Adult Autism Assessment and Diagnostic Service provides specialist diagnostic assessments for adults living in Hounslow or who have a Hounslow GP who may benefit from the clarification of a possible diagnosis of autism spectrum condition (ASC).

Cardiac rehabilitation supports patients to get back to as full a life as possible after a cardiac event, such as a heart attack, heart surgery or stent procedure.

Community matrons are highly experienced, senior nurses who provide nursing care for people who have a severe long term condition or a complex range of conditions.

The community nurse team is made up of senior district nurses, who lead a team of community nurses and healthcare assistants providing community healthcare services for house-bound patients who may well be acutely unwell, have a long term condition, require palliative care or those that require intensive and technological care.

Our nurse-led Continence Service provides an integrated bladder and bowel dysfunction service to all adult patients registered with a Hounslow GP.

The Community Recovery Service is an integrated health and social care service for adults which supports independence by providing rehabilitation and reablement after an acute illness, injury or change in life circumstances.

The Diabetes Service in Hounslow provides support for Diabetes management and specialist support In-GP practice, telephone advice line and email advice service for clinicians, structured education sessions and home visits for housebound clients.

We are working with Hestia and the London Borough of Hounslow to provide an Enhanced Dementia Care Service to local residents in the borough.

The Hounslow Falls Prevention and Bone Health Service aims to reduce the rate and risk of falls and fragility fractures amongst Hounslow adults, and to improve the health and mobility of older people and sustain their independence.

This specialist service accepts referrals for the treatment of hand injuries from Hounslow GPs and local hospitals / walk-in centres.

The Integrated Community Response Service (ICRS) aims to prevent people from being admitted to hospital if they don’t need to be. When someone does need to be treated in hospital, we help them to be discharged as soon as possible to continue their care at home.

Our intravenous therapy service is an integrated community service led by clinical nurse specialists in intravenous (IV) therapy.

The MS Service is provided as part of the Hounslow Community Recovery Service. The service provides specialist therapeutic and practical support to people with MS, their families or carers at the time they need it most.

Our adult nutrition and dietetics service is a team of registered dietitians who provide one-to-one telephone, video, face to face and home visit consultations for a wide range of problems including gastrointestinal issues, malnutrition, weight management and home enteral feeding.

Our Pain Management service provides assessment, treatment and management advice for clients presenting with a wide range of chronic musculoskeletal pain conditions

The Parkinson's Service is a community-based team with specialist knowledge of all aspects of the condition, comprising nursing therapy and pharmacy support.

The domiciliary phlebotomy service takes routine and urgent blood tests in the community for housebound adults aged 16 or over. Blood pressure measurements are taken if requested by the GP only

Physiotherapy helps people affected by injury, illness or disability through movement and exercises, manual therapy, education and advice (The Chartered Society of Physiotherapist).

We are a team of speech and language therapists with specialist knowledge in acquired neurological conditions and voice disorders.

Single Point of Access is a one-stop referral system for patients residing in the London Borough of Hounslow
