The Hounslow Community Respiratory Service provides specialist care for adults with respiratory conditions in the community. Our aim is to deliver high quality, patient centred respiratory care that supports individuals to live well with their condition and maintain independence.

We support patients to manage their symptoms, improve their quality of life, and maintain independence. The service is delivered by a multidisciplinary team working closely with GPs, hospital teams, and other community services. Care is provided in a range of settings including clinics, community venues, patients’ homes, and virtual platforms.

We provide ongoing support for patients with chronic respiratory conditions, including COPD, complex asthma, bronchiectasis, and interstitial lung disease.

The service includes:

  • Personalised care planning
  • Symptom monitoring and early intervention
  • Self-management support and education
  • Coordination with GP and hospital services
  • Early supported discharge from hospital, where appropriate

The aim is to support patients to remain stable in the community and reduce avoidable hospital admissions.

Pulmonary Rehabilitation (PR) is a structured programme for people with long-term lung conditions such as COPD, bronchiectasis, and interstitial lung disease.

The programme includes:

  • Supervised exercise sessions
  • Education on breathlessness management
  • Advice on pacing and energy conservation
  • Support to improve confidence in daily activities

PR is delivered via:

  • Community-based group programmes
  • Virtual (online) sessions
  • Home-based options where clinically appropriate

Pulmonary Rehabilitation can help improve exercise tolerance, reduce breathlessness, and support self-management.

The service provides specialist assessment and ongoing review for patients who may require oxygen therapy.

This includes:

  • Assessment for long-term oxygen therapy
  • Ambulatory oxygen assessment (for use during activity)
  • Monitoring and review of oxygen usage
  • Patient education on safe use of oxygen
  • We work closely with primary and secondary care teams to ensure oxygen therapy is appropriate and regularly reviewed.

We provide chest physiotherapy for patients who require support with secretion clearance or symptom management.

Interventions may include:

  • Airway clearance techniques
  • Breathing exercises
  • Advice on managing cough and sputum
  • Support during and following respiratory infections

The service is available to:

  • Adults aged 18 years and over
  • Patients registered with a Hounslow GP
  • Patients with a diagnosed or suspected respiratory condition

Referral criteria

Referrals are accepted for patients who:

  • Have a confirmed or suspected respiratory condition
  • Experience breathlessness impacting daily activities
  • Require pulmonary rehabilitation
  • Require oxygen assessment or review
  • Need chest physiotherapy or airway clearance support
  • Require follow-up after a respiratory-related hospital admission
  • Are you able to engage with the service (face-to-face, virtual, or home-based)

Referrals may not be appropriate where:

  • The primary condition is non-respiratory
  • The patient is medically unstable and requires acute care
  • The patient is unable to participate in assessment or treatment and no support is available
  • Risks cannot be safely managed within a community setting
  • The patient is already under an alternative specialist service providing equivalent care
