Community matrons are highly experienced, senior nurses who provide nursing care for people who have a severe long term condition or a complex range of conditions.

Typically these will be people who have had several emergency hospital admissions or are at risk of having to move from their own home into a nursing home because of their condition.

Community matrons have advanced clinical skills in physical examination and assessment, clinical decision making and can prescribe a wide range of medication and work closely with other services and your doctor to plan and organise your care.

They will act as your ‘case manager’ – a single point of contact for care, support or advice, offering efficient and quicker access to clinical treatment to prevent hospital admission and to ensure that when hospital admission is required there is support to assist an earlier discharge.

The community matrons work alongside the community nursing service who provide some of the care individuals need alongside and with oversight from the matron.

The community matrons visit patients in their homes. They also provide advice and support via telephone consultation.  

Referrals

All referrals to the Community matrons should be made using the HCH referral form through the Single Point of Access service (SPA).

If you're a new patient please contact your GP who can refer you to the service via the Single Point of Access Service (SPA).

If you're patient that has previously been on our caseload, you can contact the service and ask for a nurse who will assess if can accept you back to the service via self-referral.
