The Community Recovery Service is an integrated health and social care service providing rehabilitation and reablement after an acute illness, injury or change in life circumstances.
It aims to restore and maximise physical abilities to support independence in daily life through episodes of rehabilitation and reablement.
Referral criteria
To access the service, people must:
- Be aged 18 or over
- Require rehabilitation and / or reablement after an acute illness, injury or change in life circumstances
- Be registered with a Hounslow GP and / or resident in London Borough of Hounslow (Reablement pathway requires the person to be a resident of the London Borough of Hounslow)
Referrals
Referrals to the service can be made by GPs or other healthcare professionals, hospital teams, social services, or the voluntary sector. People can also self-refer to the service.
Referrals from all professionals
Referrals can be made via the Hounslow Single Point of Access (SPA) Single Point of Access (Hounslow) service. All referrals need to be completed on the Single Point of Access referral form.
Referral forms can then be emailed across to SPA:

Triage of referrals is supported by the inclusion of relevant discharge summaries, clinic letters and reports.
Any health and social care professional or voluntary agency can refer people to this service.
