The Hounslow Enhanced Dementia Care Service is an integrated health, social care and voluntary sector service delivered by West London Trust working with Hestia Housing and Support and the London Borough of Hounslow. We support individuals and their care or support network to enable them to live well with dementia through pro-active interventions and care planning.
We aim to prevent or delay deterioration and reduce the risk of crisis by working with other services to provide the timely interventions and avoid hospital admission.
If you are living with dementia we can offer you a community visit to better understand you needs.
This service can then offer a range of options including community support and interventions focussing on living well and maintaining your health and wellbeing.
You will have a named contact within the service who will work with you towards a community care plan (self-management plan). We work towards self-management, meaning you and the people closest to you can maintain your health and wellbeing without too many health or care services involved. EDCS is always available should your needs change and self-referrals are encouraged at any stage.
The service is staffed by Nurses, Coordinators, Occupational Therapists, Assistants and Administrators drawing on experience across health, social and voluntary sectors.
Service Criteria and Referrals
Please note, EDCS is not a diagnostic service and all person’s referred must have a confirmed diagnosis of dementia. We cannot accept referrals for people with a mild cognitive impairment.
The person must:
- have a confirmed diagnosis of dementia
- live within the borough of Hounslow and/ or have a Hounslow GP
- live within their own home
- be aged 65 and over (consideration will be given to those under 65 on a case by case basis)
Referrals will be accepted from all professionals via the Single Point of Access. We encourage any relevant supporting documents as part of the referral.
