Triangle-of-Care.pngThe Triangle of Care is a partnership between professionals, the person being cared for, and their carers. It sets out how they should work together to support recovery, promote safety and maintain wellbeing.

These are the six standards of the Triangle of Care:

  1. Carers and the essential role they play are identified at first contact or as soon as possible thereafter
  2. Staff are ‘carer aware’ and trained in carer engagement strategies
  3. Policy and practice protocols regarding confidentiality and sharing information, are in place.
  4. Defined post(s) responsible for carers are in place
  5. A carer introduction to the service and staff is available, with a relevant range of information across the care pathway
  6. A range of carer support services is available.

We’re proud to be awarded Triangle of Care Star 2 accreditation. This is a national award recognising organisations that continually work towards embedding partnership working with carers across their services.

The award is part of the ‘Triangle of Care’ quality improvement programme, developed by carers and delivered by Carers Trust. It supports health and social care organisations to improve safety, recovery and wellbeing by ensuring carers are recognised as partners in care, and supported through six key “carers included” standards.

The Trust has been commended for strong governance, co-production with carers and clear evidence of carer-inclusive practice.
