Rosemary ward provides specialist and neurological rehabilitation in a community-based setting which aims to help patients improve what they can do and achieve their goals for rehabilitation through multidisciplinary care. The ward is what is known as a Level 3 rehabilitation service, and is supervised by consultants in general medicine, rather than neurologists or rehabilitation specialists.

Patients are nursed in single sex bays of 6 to 8 beds.

Single side rooms are available for those patients who require specialist or isolation nursing care.

We have a large, well-equipped gym, a therapy kitchen and bathroom in which to practice daily activities before going home.

Information about the ward

Visitors are encouraged between 11am and 8pm.

Up to 2 visitors are allowed per person.

Breakfast: 8am

Lunch: 12.30pm

Dinner: 5.30pm

Patients are usually referred for inpatient rehabilitation following a hospital admission by the ward team. You can also be referred by the community rehabilitation team if they think you need more support than you’re getting at home.

The team looking after you will ask for your consent to engage in a community based inpatient rehabilitation programme.

If you agree, they’ll send a referral to the Rosemary Ward team. If you’re already known to Ealing Community Partners, they can also share your care plan.

People with an Ealing GP who live within 1 mile of the border of Ealing are eligible to be referred.

In addition, five beds are available for people with a Hounslow GP.

If you’re eligible for the service, we’ll aim to offer you an admission as soon as a bed is available.
