The Hammersmith and Fulham community independence service (CIS) offers care at home to help people avoid unplanned hospital admissions.
The team brings together the skills and experience of different health and social care professionals to provide integrated care for patients with acute illness or who need support to rehabilitate and improve their independence.
We provide short-term nursing care, occupational therapy, physiotherapy and social care to people with immediate health or functional needs.
This service is delivered in partnership with Central and North West London NHS Foundation Trust (CNWL).
While West London NHS Trust staff are employed in the Hammersmith and Fulham community independence service team, the service is led by CNWL.
