This year, the Trust will be marking World Diabetes Day on Monday 14 November.
The theme this year is to increase access to diabetes education to help improve the lives of the more than half a billion people living with diabetes worldwide.
Ealing Community Partners is part of West London NHS Trust and will be hosting a special event to mark the day on Tuesday 15 November.
The event will be taking place at Southall Town Hall from 11am to 3pm.
Healthier Lifestyle Service and Ealing Diabetes Team will be working together with Southall Community Alliance Team to raise awareness of the risk of diabetes and its complications.
The event will be a great opportunity to speak to diabetes specialist nurses, diabetes health and wellness coaches, podiatrists, dieticians and social prescribers.
There will also be the opportunity for blood glucose testing, lung age tests and CO tests, but make sure you bring a mask!
Anissa Hared, Trust diabetes and wellness coach, said: "Come and check out our services if you wish to find the best place to start and feel motivated to make some changes to manage your type-2 diabetes, big or small."
"Millions of people are affected by diabetes and although it may often seem like you're going through this alone, you most definitely are not. As a diabetes wellness coach, we have had direct experiences with people who initially felt alone but soon began making small changes to their lifestyles that went on to have a massive impact.
"The importance of eating well and maintaining your fitness level does not only concern people who have diabetes - it is something we must all consider. Get your friends and family involved in your new lifestyle change and make this something beneficial for those around you."
