Work at West London NHS Trust to improve care for older people and improve access to speech and language therapy for children has been shortlisted for three 2025 HSJ Awards.
The Trust’s dedicated team for supporting patients with dementia in care homes across Ealing, known as the Care Home In-reach Liaison service, has been shortlisted for the Transforming Care for Older People Award. It provides bespoke support for individual residents, as well as training for care home staff. Since its launch in 2024, the team has supported nearly 200 care home residents and has helped the north west London region save over £1,200,000.
In the same category is West London’s work as part of the Hounslow Borough Based Partnership for reducing delays in discharge from West Middlesex University Hospital. The partnership brings together five key Community Health services across West London NHS Trust and the London Borough of Hounslow, and supports transition of care back to primary care and neighbourhood health teams.
The Paediatric Speech and Language Therapy service, part of Ealing Community Partners and delivered in conjunction with Central and North West London NHS Foundation Trust, has been shortlisted in the NHS Race Equality Award for introducing specialist bilingual staff to support with assessments, therapy and community outreach in languages most commonly spoken in Ealing. The staff have helped to eliminate language barriers when conducting appointments and has seen an increase in the number of people who do not have English as first language at home to take up speech and language therapy.

The winners of the 2025 HSJ Awards will be announced at a ceremony on 20 November 2025 at Evolution London. The awards celebrate outstanding achievements in healthcare, promote best practice, and recognise the people and projects making a real difference to patients across the UK.
