West London NHS Trust is to be lead integrator in two boroughs helping create the conditions needed for neighbourhood health services to succeed.

The Trust will take the lead in Hounslow and Ealing working with other NHS organisations, councils and the voluntary and community sector to build a shared understanding of local need and support that is more proactive, connected and centred around people and neighbourhoods.

It follows an announcement from NHS North West London  that will continue to move neighbourhood health forward, with integrator arrangements now confirmed across all eight boroughs in its area.

This marks an important step in strengthening how local partners work together to deliver more joined-up, community-based health and care.

Working alongside existing borough partnerships, integrators will focus on improving collaboration, learning from what already works well locally, and helping create the conditions needed for neighbourhood health services to succeed.

The approved borough integrators which include West London NHS Trust are:

  • Ealing: A partnership with the London Borough of Ealing, London North West University Healthcare NHS Trust, Ealing and Hounslow community and voluntary sector and Ealing primary care. West London NHS Trust will host the arrangement.
  • Hounslow: A partnership with Hounslow council, the Hounslow GP Consortium and Chelsea & Westminster Hospital NHS Foundation Trust. Again West London NHS Trust will host the arrangement.
  • Hammersmith & Fulham: A partnership with CLCH, Chelsea & Westminster Hospital, Hammersmith & Fulham PCNs, Imperial College Healthcare NHS Trust, Hammersmith & Fulham council and Sobus. CLCH will host the arrangement.

These arrangements do not replace existing partnerships. Instead, they provide additional leadership, coordination and support to help neighbourhood health services develop at pace and scale, improving outcomes and reducing inequalities for local communities across North West London.
