Ealing’s community health teams joined forces at the MSK Trailblazer Community Appointment Day event.
 

On Thursday 3 July, five of Ealing’s frontline health teams came together for a trailblazing Musculoskeletal (MSK) community health day.

This collaborative event, part of a wider NHS initiative, focused on supporting people with musculoskeletal conditions, by offering joined-up clinical care and wellbeing support. The Diabetes Service, Healthy Ealing Team (HET), Ealing Smokefree Service, Tuberculosis (TB) and Immunisation and the Diabetes Health & Wellness Coaching Team were all present, with the aim of helping individuals regain their independence and offering holistic clinical and wellbeing support.

“This was a brilliant example of integrated care in action,” said Prachi Soni, Service Manager for Long Term Conditions. “We were able to meet people where they are and offer real, practical support that could change lives.”

Diabetes Support with a twist
The Ealing Diabetes team brought energy and creativity to the day with their interactive “Sugar Games,” helping attendees understand how different carbohydrate portions affect blood sugar. Alongside this, they shared healthy recipes and signposted people to services like Care Coordination and Right Start Education, available in English, Punjabi, and Tamil.

“Stress and pain can really impact diabetes management,” said Adamma Okwu, Diabetes Matron. “That’s why events like this are so important — they connect the dots between physical and emotional health.”

Prevention at the Heart of HET
The Healthy Ealing Team (HET) delivered a wide-ranging public health message, from raising awareness of cancer screenings to educating attendees on stroke prevention and Alzheimer’s support. They also partnered with Recovery Intervention Services (RISE) to provide guidance on drug and alcohol harm reduction.

“We want people to feel empowered to take control of their health,” said a Katerina Manginas, HET Manager. “Early detection and prevention are key.”

Smokefree: A personalised path to quitting
The Ealing Smokefree team offered their 8-week quit programme, which includes one-to-one behavioural coaching, free Nicotine Replacement Therapy (NRT), and vape-friendly cessation options. Their message was clear: “quitting smoking is one of the most powerful steps anyone can take for their health—and support is available every step of the way.”

Coaching for a Healthier Future
The Diabetes Health & Wellness Coaching Team showcased their 12-week programme, which focuses on lifestyle change, goal setting, and patient empowerment. With monthly cooking classes at the University of West London, the programme offers practical tools to help people take control of their health and wellbeing. The teams mentioned that it is more than just managing a condition, but about building confidence and creating lasting change.

The Tuberculosis (TB) service presented information on higher rates of TB in the borough and offered the community to access testing in Ealing. People were also given the opportunity to make conversations around busting myths, stigma and discrimination for continued dialogue and awareness-raising at the event.  

Together, these teams demonstrated the strength of collaboration in community healthcare, offering not just treatment, but hope, empowerment, and a path forward.
