Healthy Minds Together: Supporting Brain Health in Hammersmith & Fulham (H&F)

Healthy Minds Together is a new collaborative pilot programme led by West London NHS Trust, in partnership with Hammersmith & Fulham CouncilH&F Dementia Action Alliance, and other local organisations. The initiative aims to raise awareness of healthy brain ageing and support residents in making lifestyle changes that may reduce the risk of cognitive decline.

This free programme is open to Hammersmith & Fulham residents aged 50 and over. It is based on the 5 FINGER Research, which explores how lifestyle changes can support brain health.

Each two-hour and 15-minute workshop introduces participants to a science-based, multidomain lifestyle approach. Sessions include:

  • Brain training and memory app demonstrations
  • Nutrition advice and activities
  • Group physical activity
  • Health checks and referrals.
