Primary care plus is a service for people experiencing mental health problems who need extra support to help them recover. This support may be provided by their GP, a primary care mental health nurse or a charity sector provider such as Mind.
We support people who are being discharged from secondary care mental health services, such as a community mental health teams, crisis resolution services and acute assessment teams.
We help to prevent the need for referral to secondary care by helping to prevent relapse or deterioration in people’s mental health.
- Help that is close to home and in the familiar surroundings of your GP practice.
- Improved communication between the people responsible for both your physical and mental health, which improves the quality of the service you receive.
- A more seamless transition back to the community for people discharged from specialist mental health services.
We can help you identify your needs, manage your mental health and help you find practical solutions to life’s challenges. If appropriate, we may tell you about other services that you might find useful.
