Community mental health survey

Each year, we take part in the Care Quality Commission’s (CQC) national community mental health survey. The CQC collects information from people who use our adult community mental health services and we use the feedback we receive to inform improvements to our work.

SurveyIf you are over 18 and used our services between 1 September and 30 November 2015, you may be asked to take part.

If you are selected to take part, you will receive a survey from Quality Health, a third-party organisation which is managing the survey on our behalf.

Your feedback is really important as it helps us to evaluate, monitor and improve the services we provide. Taking part is voluntary, and all answers are confidential.
