Enhanced Primary Care in care homes  

This is a primary care (GP) service for people in nursing homes in the Ealing area. We provide routine medical and urgent care from 8am to 8pm, 7 days a week and 365 days a year.


About the service


The service looks after patients in 16 nursing homes across the borough of Ealing ranging from smaller homes with 22 beds, to the largest home of 165 beds. We look after all age ranges, end of life patients, patients with dementia, comorbidities and patients with learning disabilities.


We work alongside other community practitioners from West London NHS Trust and Marie Curie to provide a range of assistance with care planning and to support patients to receive the care they require in their usual place of residence wherever possible.


In addition to general GP services, we proactively visit the care homes with extended services which support investigations, care planning, medication management and seasonal vaccinations.

Information about your care 


Hours of operation


The core opening hours for the service are 8:00 am – 8:00 pm, every day of the year. 

There is a single phone number to call for all enquiries, clinical or administrative. 

Designated regular visit days are agreed between the Nursing Home managers and the GP.  Variation of these visits may occur around bank holidays.


Surgery appointments are booked via the same single point of access.


Outside of the hours of 8am – 8pm patients can access advice and support by calling 111 (111*6 for nursing homes). 111 is able to arrange out of hours visits from nursing or medical staff if required. Ealing CCG (on behalf of NHS England) is responsible for commissioning out of hours services.


Obtaining repeat prescriptions and dispensing arrangements


If you are on regular medication we will work with care home staff to arrange repeat prescriptions with the doctors’ agreement.


Prescriptions typically take >2 working days (Mon-Fri) following the day of submission in order to be processed. Requests should be made in writing via the contact methods above. The Enhanced Primary Care in Care Homes Service does not dispense medication.

Referrals


The nursing home will register new patients with the enhanced primary care service for care homes by supplying the relevant documentation.
