Community and District Nursing Service

Our community and district nursing service in Ealing provides you with nursing care if you have a condition that means you cannot leave home. 



About the service


Our Community and District Nursing team in Ealing  provide you with nursing care if you are:


  • over 18 years old
  • have a short or long-term condition which makes it difficult for you to leave the house
  • would find it difficult to access regular healthcare if we did not visit you. 

We provide our core service seven days a week between 8am and 8pm with additional support provided outside of these hours through our team integrated with Rapid Response.


If you're under our care, you will have an team of nurses to call regarding your treatment or if you need help and advice.


We:

  • assess your individual healthcare needs

  • discuss and plan your care with you

  • provide you with skilled nursing care in your own home

  • refer to to other agencies if you need.

When you call us, we will ask you several questions. This is so that we can identify you, and your reason for calling.  We will then pass your call on to your district nursing team or advanced community practitioner straight away.





As well as information about your care we provide you with:


  • general nursing care when you are ill
  • ongoing nursing care after you have left hospital
  • advice on how to live a more healthy life
  • advice on continence
  • administration of medication
  • care for you if you have a long-term condition and complex needs
  • screening
  • immunisation
  • palliative and end-of-life care
  • assessment for equipment so that we can keep you as independent as possible
  • referrals to other health and social care services if you need.

Our team also provides blood tests (phlebotomy) for patients who are unable to visit the GP practice or hospital.


We work very closely with your local GP, and work in partnership with other health and social care professionals.


You can be referred to the District Nursing service by: 


  • your GP
  • your Practice Nurse
  • a hospital
  • other health and social care professionals.

Where we provide your care


We provide care in your own home.


Our teams are based in three locations across Ealing:

  • Acton Health Centre, 35-61 Church Road, Acton, London W3 8QE *temporarily based in Sycamore Lodge, Acton
  • Featherstone Road Health Centre, 2a  Hartington Road, Southall UB2 5BQ
  • Greenford Green Clinic, 3 Wadham Gardens, Greenford UB6 0BP
