Jasmin Ward provides general rehabilitation in a community based setting which aims to help patients improve their abilities and achieve the rehabilitation goals, through planned nurse and therapy led care.

Information about your care 

Most patients are nursed in single sex bays of 6- 8 beds.

Single side rooms are available for those patients who require specialist or isolation nursing care.

We benefit from a large, well-equipped gym which includes a therapy kitchen and bathroom to practice daily activities prior to going home.

Visitors are encouraged between: 11am to 8pm

Maximum visitors per person: 3

Ward routine

Breakfast: 08:00

Lunch: 12:30

Dinner: 17:30


Patients may be referred for inpatient general rehabilitation following a hospital admission by the ward team, or if you are receiving community rehabilitation and identified as requiring a greater level of support by one of the team members.

The hospital team looking after you, will ask you if you consent to engaging in a community based inpatient rehabilitation programme.

Once the hospital team have this information, they will send a referral (or share your care plan, if you are already known to Ealing Community Partners) with the Jasmin ward team.

If you are eligible for the service, we will aim to offer you an admission as soon as a bed is available.
