Speech and Language Therapy Service - adults

Our Speech and Language Therapists (SLTs) specialise in assessing, diagnosing and treating you if you have a communication or swallowing disorder.

We aim to give you the best possible function, independence and quality of life by providing you with tailored treatment programmes, advice and education. 


Information about your care 


We treat patients with speech, language or swallowing difficulties, which can be associated with the following diagnosis:


  • Cardio-vascular problems such as a stroke.
  • Degenerative neurological disorder, such as Parkinson’s Disease, Multiple Sclerosis or Motor Neurone Disease
  • Learning Disabilities
  • Head injuries
  • Dementia
  • Swallowing problems due to frailty
  • Stammering
  • Respiratory conditions
  • Complications after surgery
  • Brain tumour
  • Other cancers

Community service

  • We provide locality-based, outpatient clinics.
  • We provide domiciliary visits to housebound patients.
  • We provide outreach support for patients in other non-hospital settings, such as nursing homes and hospices.

Support for primary care and nursing homes

  • We provide specialist advice and guidance to GP’s, Nursing Home staff, patients and carers via clearly publicised communication channels, including involvement in discussions about risk.
  • We flag ‘high risk’ patients to GPs/nursing home staff for potential follow up and close monitoring.
  • We provide education and training on specific pathology (such as aspiration pneumonia) and strategies for managing risk in people with swallowing difficulties.

Community Team for People with Learning Disabilities

  • We work closely with Service Users and Carers to support Adults with Learning Disabilities to fulfil their potential and to improve their quality of life.

We work closely with other Health Care Professionals, GP’s, Nurses, Dietitians, etc and offer training and education in supporting you with communication and swallowing difficulties.


Care for Inpatients – Clayponds Rehabilitation Hospital

We offer rehabilitation assessment, therapy and treatment, in conjunction with the wider medical team, if you are staying at Clayponds Hospital.

Referrals


Our outpatient and community services in Ealing will accept referrals from GP’s, Consultants or self-referrals.  To refer, please fully complete our referral form and email to:

LNWH-tr.outpatientspeechtherapyreferrals@nhs.net.


Patients with Learning Disabilities will require a referral to the multi-disciplinary team at Green Lane.
